Good Faith Estimate

 

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.

 

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

 

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

  • Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call 800-985-3059.

FEES & PAYMENTS

Sessions: 

 

I offer sessions virtually in the State of Washington using Simple Practice Telehealth, a HIPAA compliant platform. Sessions can be held weekly or every other week. Scheduling options are discussed in greater detail during your initial consultation.

 

My fees are:

  • $150.00 evaluation.

  • $130.00 60 minute follow up session.

  • $130.00 Family session 1 hr long.

  • $100.00 45 minute follow up session.

  • $75.00 30 minute follow up session.

  • When you use your insurance and I am a provider in-network, you are responsible for your copay, coinsurance, and deductible as specified by your insurance plan.

  • Please do not hesitate to inquire about options to access counseling when you are not insured and are unable to pay full fees.

Payment Options:  Cash, Check, and credit cards (Visa, Mastercard, Discover and American Express). Payments can easily be made using the client portal or mailing a check.
 

Insurance: Currently, I am accepting First Choice, Premera, Kaiser Permanente PPO and HMO, Lifewise, Blue Cross, Blue Shield and Regence Blue Shield, Aetna, HMA, insurance plans.
Since insurance plans tend to vary, please make sure you contact your insurance to check about coverage.

Unfortunately, at this time I am not a provider for United Behavioral Health, Cigna, Medicare or Medicaid CHPW.
